Hockey ACT Community Heroes Nomination Form

In 2023 Hockey ACT introduced a new class of membership to recognise those players, coaches, umpires and officials who have made an indelible contribution to the Hockey ACT through their service and dedication to the ACT hockey community. This award has been established to recognised community contribution, not the recognition of high-performance contribution.

Minimum of 500 games or 20 year playing in the Hockey ACT competition

Consistent contribution as an umpire over at least 20 years

Consistent contribution as a coach over at least 20 years

Consistent contribution as an official over at least 20 years
